This Telehealth Informed Consent (“Consent”) applies to all telehealth services provided by Endless Vitality, LLC (“Endless Vitality,” “we,” or “us”). By scheduling or participating in a telehealth consultation, you acknowledge that you have read, understood, and agree to this Consent.
What Is Telehealth?
Telehealth is the use of secure electronic communications, such as live video or telephone, to enable a licensed healthcare provider and a patient in different locations to interact for the purpose of evaluation, consultation, treatment, and follow-up care. A telehealth visit does not include a hands-on, in-person physical examination.
Nature of Our Services
Endless Vitality offers hormone optimization, testosterone replacement therapy, peptide therapy, and medically supervised weight loss through telehealth. During your visit, a licensed provider will review your medical history, intake form, and any lab work, discuss your goals, and determine whether treatment is clinically appropriate. A provider may, in their sole clinical judgment, decline to prescribe treatment or recommend in-person care.
Potential Benefits
Telehealth can improve access to care, reduce travel and wait times, and allow timely evaluation and follow-up from the convenience of your home.
Potential Risks and Limitations
As with any medical service, telehealth has limitations. Because a provider cannot perform a hands-on physical examination, certain conditions may go undetected. Technology failures, delays, or poor connection quality may affect the visit. Although we use secure systems, no method of electronic transmission is completely secure. In rare cases, a lack of access to your complete records could affect clinical decisions. Your provider may determine that telehealth is not appropriate for your situation and recommend an in-person evaluation.
Provider Licensure and Applicable State Law
Telehealth services are provided by clinicians licensed in the state where you are physically located at the time of your visit. Your consultation, the establishment of the provider-patient relationship, and any care you receive are governed by the laws of that state. You agree to provide your accurate physical location at the time of each visit.
Prescriptions
Any prescription is issued solely at the discretion of your licensed provider and only when clinically appropriate. Prescribing of controlled substances through telehealth is limited by, and subject to, federal law (including the Ryan Haight Act and U.S. Drug Enforcement Administration rules) and applicable state law. No prescription, treatment, or specific outcome is guaranteed.
Confidentiality and Privacy
Your personal and health information is handled in accordance with applicable privacy laws, including HIPAA where applicable, and our Privacy Policy. We use reasonable administrative, technical, and physical safeguards to protect your information.
Your Rights
Your participation in telehealth is voluntary. You have the right to ask questions at any time, to request an in-person referral, and to withhold or withdraw your consent to telehealth at any time without affecting your right to future care or treatment. You may also request a copy of this Consent and any applicable state-specific notice.
Telehealth Is Not for Emergencies
Telehealth is not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use this website or our telehealth services to seek emergency care.
State-Specific Telehealth Notices
Endless Vitality currently offers telehealth services in 48 states. Telehealth laws, informed-consent requirements, and prescribing rules vary by state, and the notice that applies to you depends on the state in which you are located at the time of your visit. Some states require, among other things, specific consent language, a synchronous (live) video visit before prescribing, additional disclosures about provider identity and licensure, or particular limits on the medications that may be prescribed via telehealth.
By proceeding, you acknowledge that your visit will comply with the telehealth requirements of your state, and that any state-specific terms required by your state’s law are incorporated into and form part of this Consent.
